Working with hospital staff to understand your care requirements

We Start Early and Stay Involved

We don’t wait for discharge to get involved. We start earlier - often while you’re still in the ward.


We can:


  • Come to the hospital and meet you

  • Talk to staff and coordinators to understand your needs

  • Train family or staff in new care skills (like PEG feeds)

  • Make sure your home or STA home is ready


  • Be there on day one - and keep checking in after
